State prosecutors find ‘troubling’ behavior but pass on bringing criminal charges against former Unified Fire Authority chiefs

But in interviews with 38 witnesses, the office's investigative report on the case found the way UFA was structured "created an environment in which there was little oversight or control of the officials’ actions."
